1 Year Contract Assistant / Care Coordinator [WEST] #HZTZ
📍 Location: must be open to deployment in the west
💰 Salary: Up to $3,000
🕒 Working Hours: 5.5 Days Work Week
Job Responsibilities
• Coordinate and support patient care activities as part of a multidisciplinary healthcare team.
• Build and maintain relationships with patients, caregivers, and healthcare professionals.
• Assist patients in accessing healthcare resources, community services, and support programmes.
• Perform basic health parameter measurements and document patient information using IT systems.
• Follow up on preventive health screenings, vaccinations, and national healthcare initiatives.
• Provide basic health education on lifestyle management, chronic disease prevention, and healthy living.
• Guide patients on the use of health monitoring devices such as blood pressure monitors, glucometers, and weighing scales.
• Support social prescribing initiatives to encourage social and physical activities.
• Maintain accurate records and ensure timely documentation of care activities and clinical indicators.
• Leverage technology platforms to improve patient engagement, health literacy, and access to care resources.
Requirements
• NITEC / Diploma / O-Level / A-Level qualifications.
