Position Summary: |

|
The A responsible for providing a continuum of care for guests of Wheeler Mission through case management, involving comprehensive assessment, individualized goal planning, and coordination of services to support guest stability and recovery. This role works closely with guests participating in addiction recovery programming who may also experience homelessness, mental health concerns, trauma histories, and other barriers to long-term stability. Key duties of this position include case management, crisis intervention, recovery support, mentoring, discipleship, counseling, relapse prevention support, advocacy, care coordination, and facilitating groups when needed. |
