The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

FT, Days, 8:30 AM to 5:00 PM Mon-Fri

Nurse Navigator for the Airways Disease Program at the Mary Parkes Asthma & Pulmonary Center. will work closely with our pulmonary specialists to deliver comprehensive care to our Asthma, COPD, and Bronchiectasis patient populations, will be crucial in guiding patients through their care journey. Coordinating the entire care delivery process for Asthma, COPD, and Bronchiectasis patients, starting from their initial referral. Arranging post-hospital visits, providing essential education, promoting adherence to treatment plans, and connecting high-risk patients with home care resources. Acting as a liaison for the prior authorization process for specialty medications and necessary equipment. Developing and maintaining a registry specifically for high-risk patients within the Center. Attending bi-monthly Airways Disease conferences to stay current and contribute to program development. 3 years of experience, ambulatory experience preferred.
