Salary : $ 60,000- $ 65,000/ Year Depending on years of Experience

PURPOSE:
This position is responsible for providing leadership, direction and training to the case management team for adult residential services programs, ensuring appropriate assignment of caseloads (e.g. matching staff strengths to consumer needs), overseeing the implementation strategy for person-centered planning, and for ensuring successful collaboration with the health and wellness, clinical and behavioral, day program case management, and independent service coordination team(s).

ESSENTIAL DUTIES AND RESPONSIBILITIES:
The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein if such duties are a logical assignment to the position.
1. Organize provision of services by coordinating the communication process with the interdisciplinary team, the development of monthly review of goals and objectives, selection and design of data collection/reports, the proper use of resource materials and equipment, and training of key individuals responsible for plan development and implementation.
a. Coordinate raw data collection for statistical reporting as required to document character of caseloads and caseload analysis as needed for program outcomes.
2. Directly responsible for collaborating with all centralized support team leadership/directors, independent service coordination agencies, and outside referral sources to ensure safe, functional, and therapeutic integration of programming based on best practices in the field.
a. Collaborates with the health and wellness team/administrative assistant to ensure that all residents’ appointments are maintained and attended and ensures all residents have necessary adaptive equipment and assessments required to meet these needs.
b. Collaborates with the clinical/behavioral team to ensure successful delivery of interventions and ensures all residents have necessary augmentative and/or adaptive equipment/tools, and assessments required to meet these needs as outlined in all service delivery plans.
c. Maintains oversight of department tracking systems to ensure above coordination & service delivery, including but not limited to (parent/guardian contact information, ISC/ISSA assignment/contact information, scheduling/completion of all required annual & follow-up medical appointments, annual and subsequent IDT/PCP meetings).
d. Establish and maintain relationships with funding and referral sources, and other providers in order to expand and enhance supports to individuals served.
e. Serve as a resource to staff regarding processes related to the case management function (e.g. referral, assessment, progress review, behavioral strategies).
3. Stays abreast of best and evidence-based practices for adults with ID/DD in the provision of skill acquisition, physical, behavioral and mental health. Oversee core/therapeutic services that may include health and wellness, creativity, communication, quality relationships, and community inclusion.
4. Directly accountable for ensuring the delivery and/or coordination of all case management services are in compliance and accordance with policies and procedures promulgated through agencies responsible for surveillance and regulatory oversight of Little City’s residential services including: BALC, DPH, DHS Rules 115 and 116, HCBS, CARF, HUD, and any other entity to which the adult residential programs maintain accountability.
a. Oversee the coordination of incident report documentation as it relates to policy, DHS regulations and CARF requirements including review of reports, maintaining a database and coordinating data collection for statistical outcomes.
b. Perform regular audit reviews for reporting compliance ensuring accurate documentation and filing of information in case records.
c. Serves as department liaison for coordinating audits from regulating agencies with the assistance of the Director of ARS.
d. Oversee the collection of accurate information which may include: all rate sheets, ICAPs, 1010 forms, 60D and 67D packets, nightly absences, billing, program outcomes, and grant information as requested.
a. Directly responsible for maintaining collaboration between the finance department and oversight of financial information required for reimbursement of services.
e. Maintain confidentiality of all billing, program demographic and personal information per HIPAA and the Confidentiality Act.
5. Act as a liaison between the agency and family members, stakeholders, professional connections and community resources.
a. Communicate regularly, contingent upon individuals’ consent, with family/guardian/designees to relate progress, achievements, concerns, or unusual incidents.
b. Effectively communicate to all levels any issues, needs and problems concerning the program to the direct supervisor, administrator and assigned staff.
c. Advocate for individuals served and bring to light any barriers to independence.
d. Assist in goal/objective defining with family, staff, and other applicable outside agency personnel.
6. Maintain reduced caseload as needed, serving as case manager for designated individuals and act as interim case manager when direct reports are on approved time off.
7. Directly responsible for the training, on-boarding, and ongoing evaluation of all case managers regarding duties, best practices, policies, and procedures.
8. Provides recommendation for admission, discharge, and/or transfers as part of the Admissions Committee.
9. Assists the Director of ARS in aligning program vision and mission with best/evidence-based practices for population(s) served.
10. Assist in the monitoring of recommended staff to client ratios including suggestions regarding additional support.
11. Oversees case managers on-call and provide 24/7 emergency coverage as needed.
12. Coordinates, assists with, and actively participates in department meetings and other meetings as required by supervisors.
13. Coordinates, assists with, and actively oversees the professional development of supervisees.
14. Serves on various agency committees as desired or assigned.
15. Successfully completes Little City required training, re-training, and any additional training as required by Little City policies and practices, or as assigned by his or her immediate supervisor. Actively participates in any professional development opportunities, as appropriate.
16. Maintains annual American Heart Association certification in First Aid and CPR.
17. Perform any other reasonable duties as assigned.
