Assistant Nurse Clinician (Community / Long term care) #HJO

Job Description:

• Patient Care & Monitoring: Provide personalized care, assess recovery, and escalate concerns as needed.
• Admissions & Discharges: Manage patient admissions, transfers, and discharge planning.
• Team Collaboration: Work with colleagues to implement evidence-based treatments for optimal outcomes.
• Medical Rounds & Procedures: Participate in rounds, execute physician orders, and assist in procedures.
• Communication & Problem-Solving: Update the team on patient progress and support clinical challenges.
• Patient Education & Resource Management: Educate patients and manage resources efficiently.
• Infection Control & Safety: Monitor infection prevention practices and emergency response efforts.
• Training & Development: Train and supervise new nurses and students while supporting department initiatives.

Job Requirement:

• Min Degree in Nursing
• Min 3 yrs of relevant perm inpatient & senior nursing experience
• Preferred in Advanced / Specialist Diploma in Geriatric / Community Nursing
