Assistant Nurse Clinician (Nursing Home/AMK/ 3 Shifts)

Our Client is a reputable nursing home and is looking for ANC to join the team.

  • Provides comprehensive patient care in partnership with the multidisciplinary team(MDT), based on individual assessments and healthcare needs.
  • Coordinates and manages patient admissions, transfers, and discharge processes.
  • Monitors and evaluates patients’/residents’ responses to care plans, adjusting interventions as necessary.
  • Works closely with the MDT to deliver evidence-based nursing care and achieve optimal patient outcomes.
  • Observes patients/residents for any changes in health status, identifies early indicators of clinical deterioration through appropriate assessments, and escalates concerns promptly for timely intervention.
  • Actively participates in multidisciplinary rounds and ensures medical orders, treatments, and investigations are carried out accurately and efficiently.
  • Assists physicians during bedside procedures and therapeutic interventions.
  • Communicates patient/resident progress and treatment responses effectively with the MDT to support coordinated care planning.
  • Applies sound clinical knowledge, critical thinking, and professional judgment in delivering safe and effective nursing care.
  • Supports the nursing team in resolving patient, resident, clinical, and operational issues, seeking guidance from the Senior Nurse Manager (SNM) or Nurse Manager(NM) when required.
  • Provides education to patients and families regarding disease management, prevention strategies, rehabilitation, and self-care practices, while assessing their understanding and readiness to participate in care.
  • Contributes to cost-effective resource utilization and assists supervisors in maintaining departmental expenditures within approved budgets.
  • Evaluates infection prevention and control practices, monitors compliance with established standards, and recommends improvements where necessary.
  • Organizes and delivers educational sessions for nursing staff and students to enhance clinical competencies and promote quality patient care.
  • Supports the implementation of new nursing initiatives and change management activities in collaboration with relevant stakeholders.
  • Assesses the clinical performance of newly employed nurses and nursing students, offering constructive feedback and documenting progress for appropriate review.

Requirements:

  • Minimally 2 years of working experience as SSN1
  • Able to perform 3 rotating shift duties
