About the Role

Integrate behavioral health care into primary care settings to make mental health support more accessible, coordinated, and effective for patients. As a Behavioral Health Care Manager, you'll support individuals experiencing common mental health and substance use concerns by providing brief, evidence-based interventions while working closely with medical providers and psychiatric consultants as part of a collaborative care team.

This role is ideal for a behavioral health professional who thrives in a fast-paced, team-based environment and is passionate about bridging the gap between physical and mental health care. You’ll manage a defined patient caseload, conducting screenings, providing short-term interventions, and tracking treatment progress over time to ensure patients are improving and staying engaged in care. Working in close coordination with physicians, psychiatrists, and other behavioral health providers, you'll help guide treatment decisions, support care transitions when needed, and connect patients with additional resources. Your work will directly improve access to mental health care within primary care settings and strengthen outcomes across both physical and behavioral health.

Location

  • Office Location: Grand Rapids, MI
  • Main Service Area: Grand Rapids and surrounding areas
    • This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

  • Monday to Friday, 8 a.m. to 5 p.m.

Required Qualification

  • Formal education or specialized training in behavioral health (social work, nursing, or psychology)
  • Minimum 2 years of social work or behavioral health experience
  • Experience screening for common mental health and substance use disorders
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Essential Skills and Competencies

  • Working knowledge of differential diagnosis for common mental health and substance use disorders
  • Experience with assessment and treatment planning for mental health and substance use concerns
  • Proficiency in evidence-based psychosocial treatments and brief behavioral interventions
  • Strong therapeutic engagement skills with ability to build rapport quickly
  • Experience with evidence-based counseling techniques
  • Basic knowledge of psychopharmacology for common mental health disorders (within scope of practice)
  • Ability to work effectively in collaborative care team model
Pay Range
$55,000$62,000 USD
