About the Role
As a Partner in Wellbeing, the Behavioral Health Practitioner provides rehabilitative services to clients with mental illness or children with emotional disturbance in the form of instruction, support, and assistance for the purpose of promoting psychiatric stability, social competencies, personal and emotional adjustments, and independent living skills. Behavioral Practitioner, under the oversight, direction, and evaluations of a mental health professional, may provide an adult client with client education, rehabilitative mental health services, functional assessments, level of care assessments, and treatment plans.
Essential Responsibilities
- Provides services as outlined in the client’s treatment plan in the home, community, school, or office to help clients cope with and recover from the symptoms of their mental health condition.
- Practitioners may provide services to individuals, couples, families, or groups.
- Provide adequate documentation of services.
- Provide Targeted Case Management to individuals with serious and persistent mental illness and/or severe emotional disturbance as determined.
- Complete initial and ongoing mental health outcomes measurements as well as develop and implement treatment plans.
- Complete and update intake paperwork, screeners, functional assessments, releases, and forms/paperwork as needed.
- Assist clients, as needed, in successfully completing activities that support structured daily living.
- Participate in clinical supervision and team meetings as well as Interdisciplinary care coordination meetings.
- Schedules time effectively to ensure assigned billable hours targets are met.
- Participates and works collaboratively in all care coordination activities concerning clients on caseload as indicated by the care coordination managing the care-coordination function.
- May provide crisis assessments and interventions.
- Focuses on integrated health (physical, mental, substance use, and oral) care, ensuring clients are referred to any necessary internal and/or external services to support their overall well-being.
- Ensures care coordination activities are client-centered.
- Participate and represent Alluma at community events.
- Performs additional duties as assigned or requested.
