Care Coordinator (Healthcare | West) #HJO
Job Responsibilities:
- Support the care team in coordinating patient care and follow-up activities
- Build and maintain positive relationships with patients, caregivers, healthcare professionals, and community partners
- Assist patients in accessing healthcare and community resources to support their health and wellbeing
- Perform basic health screening activities and parameter measurements as required
- Provide basic health education on lifestyle management, chronic disease prevention, and healthy living practices
- Educate patients on the use of health monitoring devices such as blood pressure monitors, glucose monitors, and weighing scales
- Support care coordination activities aimed at improving patient outcomes and reducing hospital admissions
- Assist with social prescribing initiatives and community engagement programmes
- Maintain accurate records and ensure timely documentation of patient interactions and interventions
- Perform other duties and projects assigned by supervisor
Requirements:
- Minimum O Levels, A Levels, NITEC, or Diploma qualification
- Customer service experience is preferred
- Strong communication and interpersonal skills
- Pleasant personality with a passion for patient care and community engagement
