Our RN Case Managers play a critical role in coordinating care, building meaningful patient relationships, and delivering outcomes that truly make a difference.
Description:
Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Education/License:
Diploma from an accredited school/college of nursing. Required professional licensure at time of hire.
Responsibilities:
- Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.|
- Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.|
- Carries out physician and nursing orders. Attends team meetings and care plan meetings.|
- Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.|
Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.
Other duties as assigned
The payrate for this position is from $39.00 - $54.12 per hour.
