As a Case Manager with the Housing Advocacy & Resource Team (HART), you will partner directly with individuals and families experiencing homelessness to support their journey toward stable housing and long‑term success. Through relationship‑based work, you will help participants navigate complex systems, secure housing, and maintain stability in the community.
This role is ideal for someone who believes in Housing First, harm reduction, and person‑centered care, and is motivated to walk alongside people through complex challenges with dignity and respect.

What You’ll Do
- Build strong, trusting relationships with participants through consistent, person‑centered engagement
- Maintain an active caseload and meet regularly with clients using creative and flexible outreach approaches
- Complete assessments and develop individualized, housing‑focused goal plans with participants
- Support individuals and families in locating, obtaining, and maintaining stable housing
- Coordinate care and connect participants to medical, mental health, substance use, and community resources
- Collaborate with landlords and mediate concerns to support housing stability
- Proactively address risks to housing, including eviction prevention and problem‑solving strategies
- Support participants in understanding tenant rights, responsibilities, and independent living skills
- Document services, contacts, and outcomes accurately and on time in required systems
- Work collaboratively with team members, supervisors, and community partners to support client success
