Case Manager
Ta’n Etli-tpi’tmk Position: Case Manager
Purpose:
- To collaborate with Social Development Administrators to develop case plans for social clients and their families.
- To work directly with clients and their families to create a case plan which enables the individual and family to meet their goals and aspirations.
- To work directly with the individual and family to improve living conditions and to promote self-reliance.
Key Responsibilities
Mi’kmaq Social Governance Application
- Reviews client/family application in collaboration with the SDA.
- Ensures application is complete in collaboration with the SDA.
- Collaborates with SDA to develop client/family budget based on approved rates.
Case Management
- Arranges meeting with clients to review application and goals.
- Develops case plan in collaboration with the client.
- Arranges meeting schedule with client to co-manage the case plan.
- Updates the case management plan in the MSGA system quarterly or as required and reviews the client case file with the SDA.
- Monitors client progress and provides support to client case plan goals.
Reporting
- Develops and submits timely, accurate, and comprehensive reports on client progress, program outcomes, and case management activities into the MSGA system.
- Reports on qualitative and quantitative data to provide a comprehensive picture of client progress and program effectiveness.
- Collects client success stories and testimonials to showcase the Case Management impact.
- Complies with all reporting deadlines and requirements set by funding agencies and internal stakeholders.
Client Relations
- Develops positive working relationship with clients and their families.
- Advocates for and on behalf of the client and their families.
Mi’kmaq Enhanced Social Policy
- Understands the Mi’kmaq Enhanced Social Policy and its application requirements.
- Able to explain the policy to clients and their families and other stakeholders.
- Willing to contribute to policy changes as required from time to time.
Client Privacy and Confidentiality
- Maintains and ensures client files and matters are always treated with strict privacy and confidentiality.
- Maintains secure access to program computers used to collect applicant data and information.
- Willing to sign a privacy and confidentiality agreement as part of engagement.
Data Security
- Ensures the community social department is secure and that client paper files are held in a locked and fire- proof file cabinet.
- Ensures computer and software access codes are secure and protected.
Professional Development
- Willing to participate in professional development and training activities including updates to the MSGA software as required from time to time.
- Able to travel to attend professional development and training events.
Program and Financial Compliance
- Participates in monthly program delivery compliance reviews as required with external auditors,
Supervision
- Participates in annual employee reviews with management.
- Reports to Social Development Administrators they are assigned to work with.
Qualifications & Requirements
Education & Experience
- Post-secondary education in Social Work, Human Services, Community Studies, or a related field is preferred
- Equivalent combination of education and relevant experience in social services may be considered
- Experience working with individuals and families in a case management or support role is an asset
- Experience working within First Nation communities and/or knowledge of Mi’kmaq culture is strongly preferred
Knowledge & Skills
- Knowledge of the Mi’kmaq Enhanced Social Policy or willingness to learn
- Strong case management, assessment, and planning skills
- Ability to support clients in goal setting, budgeting, and self-sufficiency planning
- Effective communication and interpersonal skills, with the ability to build trust with clients and families
- Strong organizational and time management skills
- Ability to maintain accurate and detailed records and reports
- Proficiency in computer systems and data entry (experience with case management systems such as MSGA is an asset)
