Case Manager

Job Purpose

The successful incumbent will supports the Senior/Assistant Director (Programme) and Lead Case Manager in delivering and overseeing meaningful programmes for residents of the Halfway House. It ensuresthat all programmes are effectively implemented and aligned with the HalfwayHouse’s philosophy, mission, vision, and core values to support residents’rehabilitation and reintegration.


Major Duties and Responsibilities

  • Support the delivery of rehabilitation and reintegration programmes, working with the Senior/AssistantDirector (Programme) and Lead Case Manager to ensure residents are meaningfully engaged throughout their stay at the Halfway House.
  • Safeguard programme quality and integrity, monitoring delivery and participation to ensure programmes achieve their intended rehabilitative outcomes.
  • Provide holistic case management, assessing residents’ needs and delivering timely information and referral to link residents and their families with appropriate services and community resources.
  • Guide residents in key reintegration areas, including accommodation, conduct, and employment, through home visits (where applicable), job‑site visits, and regular follow‑ups with employers to monitor work performance.
  • Facilitate group‑based correctional programmes, including psychology‑informed interventions for residents under the Community‑Based Programme (CBP).
  • Plan and coordinate outreach activities and events, working with programme partners, service providers, and volunteers to support residents’ rehabilitation and community engagement.
  • Ensure residents’ safety and well‑being during group activities, programmes, and events organised by the Halfway House.
  • Monitor and evaluate residents’ progress, contributing to internal case discussions and attending case conferences with relevant SPS officers.
  • Maintain accurate and up‑to‑date case records, ensuring all case files and documentation are completed in accordance with organisational and statutory requirements.
  • Prepare timely reports and updates, including residents’ progress, attendance, and programme participation, for reporting and submission purposes.

Any Other Duties

Carry out any other duties as assigned by the Senior/AssistantDirector (Programme) or Lead Case Manager.


Job Requirements

  • Recognised Degree in Social Sciences such as Social Work or Psychology, or other related disciplines.
  • Preferably at least three (3) years of working experience, preferably in the social service sector.
  • Team player with good interpersonal, oral and written communication skills.
  • Counselling related certification would be an advantage.
  • Proficient in Microsoft Office applications.
  • Able to multi-task, work independently and as a team player.
  • Have passion in the helping profession.
  • Level-headed and able to work with varying resident profiles
  • Keen learner who is willing to work beyond boundaries.
