Case Worker| Welfare Home
Job Description :
- Create personalised care plans tailored to the needs of the resident
- Implement care plans and regularly record residents' progress
- Design and organise programs and activities to address residents' care requirements
- Prepare and submit review reports and monthly interview records every 6 month
- Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents
Job Requirement :
- Min Diploma in Social Work/Psychology
- At least 2 year of relevant healthcare industry experience
- Working Hours: Mondays to Fridays, shift work with DO
