The Clinical Nurse Manager provides overall clinical leadership, guidance, and oversight to designated home health teams. This role is responsible for supervising, developing, and mentoring clinicians to ensure high-quality patient care, regulatory compliance, and strong clinical outcomes.
Key responsibilities include onboarding, competency evaluation, documentation review, staff development, quality improvement initiatives, and clinical program oversight. The Clinical Nurse Manager also leads efforts in quality assurance, performance improvement, and infection control.
Key Responsibilities
Clinical Leadership & Team Management
- Provide clinical oversight to home health teams, promoting best practices and exceptional patient outcomes
- Directly supervises a cohort of clinical team members
- Serve as a clinical resource for staff, patients, caregivers, and providers
- Lead interdisciplinary team meetings to coordinate care and address clinical challenges
Training, Development & Mentorship
- Coordinate onboarding and orientation for new clinical staff
- Conduct competency assessments during orientation and annual reviews
- Deliver ongoing training on best practices, new technologies, and evidence-based care
- Provide mentoring, coaching, and counseling to support staff development
Documentation & Quality Improvement
- Educate staff on documentation standards and perform chart audits for accuracy and compliance
- Analyze clinical data and key performance indicators to identify trends and improvement areas
- Lead Quality Assurance and Performance Improvement (QAPI) initiatives
- Support infection control efforts and compliance standards
Patient Care Oversight
- Provide guidance in complex clinical situations and care planning
- Support development and oversight of patient care plans and recertifications
- Collaborate with intake to ensure smooth patient admissions and transitions
- Participate in patient visits as needed (PRN) for care support or staff development
Operations & Collaboration
- Partner with leadership (Director, Quality & Compliance) to drive program goals
- Assist with patient and staff assignments as needed
- Participate in Administrator On-Call rotation
- Ensure compliance with professional standards and organizational policies
Responsibilities of Skilled Professionals
- Conduct ongoing interdisciplinary patient assessments
- Develop and evaluate care plans with patients, caregivers, and clinical teams
- Deliver services as ordered by physicians
- Provide patient and caregiver education and counseling
- Maintain accurate and timely clinical documentation
- Communicate with physicians and care team members
- Participate in QAPI and ongoing in-service training
