Role Scope:

The Clinical Services Director, reporting to the Executive Director, focuses on the management of the Habitat Health clinic, rehabilitation services, social work, and behavioral health services. This includes providing ongoing guidance and support to the Interdisciplinary Team (IDT) as a member of the leadership team.

Core Responsibilities & Expectations for the Role

  • Create a center experience that participants want to spend time in, and team members love.

  • Continue to raise the bar, including leading the clinical quality improvement process.
  • Constructively seek and share feedback and quickly implement changes in order to improve how the center and the PACE program as a whole achieve their goals and mission.
  • Establish and honor Habitat values within the team.
  • Accountable for the delivery of high-quality clinic, social work, behavioral health services, and rehab services.
  • Provide ongoing clinical guidance and support to the IDT and provide back-up planning and facilitation of the IDT meetings as needed.
  • Direct clinic and social work operations including hiring, training, supervising, and mentoring team members and ensuring clear and effective workflows.
  • Ensure participants experience smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.), deploy team members as needed, and work collaboratively with the clinical team, Operations Manager, and Medical Director.
  • Achieve key performance indicators as outlined in monthly operating reviewing and weekly tactical meetings.
  • Maintain compliance with PACE, ADHC, and DHCS regulations inclusive of audit prep, monitoring non-compliance, and implementation of corrective action plans.
  • Performs related duties as assigned, including, but not limited to filling gaps in direct participant care.

Required Qualifications:

  • Active and unrestricted Registered Nurse (RN) License in the state of California.
  • A minimum of 8 years’ experience in healthcare delivery/operations.
  • 5+ years’ experience in supervisory or administrative capacity, demonstrated aptitude to mentor and develop team members.
  • Bachelor’s degree in nursing or related field.
  • Knowledge and experience to treat participants and oversee complex clinical needs.
  • Experience in complex brick and mortar healthcare delivery.
  • Excellent communication and leadership skills.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof that all immunizations are up to date.
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.

Preferred Qualifications:

  • Master's Degree is preferred in a related field including but not limited to business administration, health care administration, nursing, social service, psychology, therapy (occupational, physical, speech, recreational), dietetics, gerontology.
  • Bilingual: Spanish preferred.
  • Experience leading multidisciplinary healthcare teams.
  • PACE, ADHC and/or value-based care experience preferred.
  • Experience in or leading case management function.

Compensation:

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $160,000-$190,000. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
