What You\u2019ll Do:<\/b>

\u2022 Provide in\-home visits<\/b> supporting patients and families.
<\/div>
\u2022 Deliver counseling, care planning, and crisis intervention<\/b>.
<\/div>
\u2022 Connect patients to community resources and support systems<\/b>.
<\/div>
\u2022 Collaborate with an interdisciplinary care team<\/b>.

What You Bring:<\/b>

\u2022 Master\u2019s Degree in Social Work (MSW)<\/b> required.
<\/div>
\u2022 Active LGSW or LISW license in Minnesota<\/b>.
<\/div>
\u2022 At least 1 year of clinical Social Work experience<\/b>.
