Pay Scale:
AP: $18-$20/hr.
QP: $23.07/hr. -$24.07/hr.
Summary:
Under direct and indirect supervision, provides case management services, person centered planning and documentation, referral and linkage, and monitoring/follow-up for clients referred to Davidson County Case Management program through Davidson County Schools (DCS).
Essential Duties and Responsibilities:
- Provides case management services to Davidson County Schools including: serving as a link between school social workers/counselors and community resources (DSS, Health Department, County, Healthcare providers, etc).
- Assist referred clients to referrals to community resources, follow up to address barriers to achieving goals, monitoring and reporting progress.
- All other duties as assigned by supervisor
The responsibilities of the Case Manager include, but are not limited to, the following:
Case Management Services
- Offer case management service to serve as a link between school social workers/school counselors and community resources (DSS, Health Department, Court, Healthcare providers)
- Referrals will be sent to care manager following a standardized process by Davidson County Schools
- Care management services will terminate after CCA if there are no recommendations for services/treatment. If CCA results in recommendations for services/treatment, care management will terminate no more than 3 months after CCA
- The program is voluntary and must be agreed upon by the parent/guardian of the child
Caseloads:
- There will be an average of thirty (30) persons served annually by each care manager. Number may vary.
Data Management/Reporting
- Case Manager will work with Daymark IT/Data Analytics Team to provide quarterly reporting on the following:
- Number being served
- Number of cases closed
- Length of time to complete CCA
- Type of treatment services established
- Tracking of school attendance and grade performance (documented and provided by school)
- Tracking of school behavioral issues (documented and provided by school).
Education and/or Experience:
A minimum of High School Diploma is required. Applicant must have a valid driver’s license. Applicant must meet NC qualifications of a Tailored Care Management (TCM) Extender OR Qualified Professional.
Extender
Must meet one of the following requirements:
- Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system. OR
- Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist. OR
- A parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (note that a parent/guardian cannot serve as an extender for their family member). OR
- Has two years of paid experience performing the types of functions described in the “Extender Functions” section below, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
- Extender Functions:

- Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation)
- Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing
- Sharing information with the care manager and other members of the care team on the member’s circumstances
- Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services
- Participating in case conferences

Qualified Professional:
Must have ONE of the following qualifications:
- A MH/SU license (including associate-level), or are certified by the NC Substance Abuse board
- A RN and have four years of full-time experience working with the MH/SU/IDD population*
- A master’s degree in a human service field AND at least one year of full-time experience working with the MH/SU/IDD population
- A bachelor’s degree in a human service field AND at least two years of full-time experience working with the MH/SU/IDD population
- A bachelor’s degree in a non-human service field AND at least four years of full-time experience working with the MH/SU/IDD population
