Enrolled Nurse/Registered Nurse (Home Care)
Job Description:
Responsibilities:
- Provide high-quality nursing care and emotional support to patients, ensuring their comfort, safety, and well-being.
- Assess patients’ clinical conditions, monitor vital signs, and administer prescribed medications and treatments accurately.
- Develop, plan, and implement individualized care plans in collaboration with the healthcare team.
- Serve home care clients through hands-on case management and conduct hospital or home visits for client assessments.
- Support the home care team with nursing cases and take up respite assignments when required.
- Advise and guide care staff in managing medical issues or emergencies.
- Communicate effectively with patients’ families, providing timely updates on care and progress.
- Collaborate with colleagues and external community partners to coordinate and manage client care needs.
- Ensure compliance with documentation standards required by the Ministry of Health (MOH) for financial and quality audits.
- Participate in complaint resolution, crisis management, and other ad hoc duties assigned by management.
Requirements:
- Minimum Nitec/Diploma/Bachelor’s Degree in Nursing.
- Advanced Diploma in Gerontology or Palliative Care is an advantage.
- Must be an Enrolled Nurse or Registered Nurse with the Singapore Nursing Board (SNB) and possess a clean disciplinary record. (Nurses on conditional licenses are not eligible for this role.)
- Minimum 2 years of clinical experience.
- Proficient in Microsoft Office and comfortable using IT tools in daily work.
- Ability to converse in local dialects will be an added advantage.
- Willingness to perform occasional shift work, particularly to support palliative care patients.
