  Primary Responsibilities:
  <\/p>

  • Perform in\-home physical therapy evaluations and mobility assessments
    <\/li>
  • Create individualized treatment plans centered on safety, strength, and functional progress
    <\/li>
  • Deliver hands\-on therapy, gait training, and home exercise instruction
    <\/li>
  • Recommend equipment and home modifications to enhance independence
    <\/li>
  • Collaborate with nurses, caregivers, and interdisciplinary providers
    <\/li><\/ul>

    Requirements:
    <\/p>

    • Active Minnesota Physical Therapist license<\/b>
      <\/li>
    • Valid driver\u2019s license and dependable transportation
      <\/li>
    • Home health experience a plus, but not required\u2014outpatient and inpatient PTs encouraged to apply
      <\/li>
    • Passion for empowering patients through one\-on\-one, goal\-driven care
