Capitated Model
Contract / Remote (US-Only)

About The Role

Mochi Medical, originally founded as an obesity management practice, is expanding its virtual care model to deliver high-touch behavior change support alongside our clinical and nutrition care. We're hiring Health Coaches to work as part of our multidisciplinary care team - alongside physicians, NPs, and Registered Dietitians - to support patients on GLP-1 medications and related programs.

Health Coaches at Mochi handle general wellness and behavior change coaching, helping patients build sustainable habits around nutrition, physical activity, sleep, stress, and emotional health. Your role is to be an effective accountability partner - helping patients consistently follow through on the plan their care team has built with them.

This role is ideal for someone passionate about metabolic health, behavior change, and leveraging digital healthcare to make a measurable difference in patients' lives.

What You’ll Do

  • Conduct 30-minute one-on-one virtual coaching sessions (initial and follow-up) via video to help patients build sustainable lifestyle habits that complement their treatments.
  • Use motivational interviewing and evidence-based behavior change techniques to help patients set SMART goals, build consistency, and navigate setbacks.
  • Carry a caseload of patients and serve as their accountability partner in making long-lasting lifestyle changes.
  • Support patients on issues like managing cravings, portion awareness, building exercise habits, stress eating, navigating social situations (vacations, holidays, dining out), and emotional dimensions of weight management.
  • Provide general education on healthy eating principles, meal planning basics, and lifestyle strategies - without crossing into Medical Nutrition Therapy or individualized therapeutic nutrition.
  • Document sessions in our EMR using SOAP format and communicate relevant updates to the care team.
  • Adapt to a fast-paced startup environment and contribute to team-wide improvements.

• Skilled in motivational interviewing and patient education.
• Proficient in online scheduling tools, EMR systems, and video-visit coordination.
• Strong understanding of scope of practice for health coaches, including the boundary between coaching and Medical Nutrition Therapy.
• Comfortable working in a fully remote, dynamic startup environment.
• Reliable high-speed internet and a private, professional video setting.

Preferred Qualifications

  • Bachelor's degree in a health-related field (nutrition, public health, exercise science, psychology, health education, or related).
  • 1+ years of experience coaching patients on GLP-1 medications, obesity, diabetes, or other metabolic conditions.
  • Familiarity with the patient experience of GLP-1 medications (side effects, titration, appetite changes, common questions) and other treatments.
  • Training in additional behavior change frameworks (stages of change, CBT-informed coaching, ACT).
