Responsibilities
Patient Assessment & Care Planning
- Conduct comprehensive initial and ongoing assessments of patients and their families.
- Develop, implement, and update individualized care plans based on physician orders and patient needs.
- Identify therapeutic, preventive, and rehabilitative interventions that support positive patient outcomes.
Direct Patient Care
- Provide skilled nursing care in accordance with physician orders and the State Nurse Practice Act.
- Administer medications, treatments, and clinical interventions as prescribed.
- Monitor patient conditions and make appropriate recommendations for care plan adjustments.
- Educate patients and caregivers on diagnoses, medications, treatments, and disease management.
Care Coordination
- Coordinate care across multiple disciplines to ensure seamless service delivery.
- Collaborate with physicians, therapists, social workers, home health aides, and community providers.
- Advocate for patient needs and help connect families with available resources.
Documentation & Communication
- Maintain accurate, timely, and compliant clinical documentation.
- Communicate patient status changes promptly to physicians and obtain necessary orders.
- Provide regular updates to the care team to ensure continuity of care.
Patient & Family Education
- Empower patients and families through education and ongoing support.
- Encourage caregiver involvement in the care planning process.
- Promote independence, safety, and improved quality of life.
Supervision & Leadership
- Instruct, supervise, and evaluate home health aides according to agency standards.
- Conduct required supervisory visits and support quality care delivery.
- Serve as a clinical leader and resource for the interdisciplinary team.
