Home Visitation Coordinator

Glenwood is seeking a Home Visitation Coordinator to join our team. This role will oversee the intake and referral processes, coordinates family engagement activities, support program operations for children and families impacted by autism spectrum disorder and related developmental disabilities.

This position plays a critical role in ensuring high-quality, family-centered services through strong coordination, communication, and data management. This is a part-time position working 20 hours per week.

Essential Functions:

  • Program Coordination: Coordinate daily Home Visitation Program operations to ensure efficient service delivery, timely communication, and compliance with program requirements.
  • Intake and Referral Management: Manage intake, referrals, enrollment, and service initiation for participating individuals and families.
  • Family Engagement and Support: Support recruitment, engagement, and retention of individuals with autism spectrum disorder and related developmental disabilities and their families; serve as a key contact for participants, caregivers, and community partners.
  • Scheduling and Service Coordination: Coordinate home visits, assessments, consultations, follow-ups, and related activities to support continuity of services.
  • Documentation and Records Management: Maintain required program documentation, intake records, consent forms, service logs, and related records in accordance with agency and regulatory standards.
  • Data Collection and Management: Collect, enter, and monitor program data in approved systems, ensuring accuracy, confidentiality, and privacy compliance.
  • Assessment Support: Assist with assigned assessments, interviews, questionnaires, and outcome measures as trained.
  • Collaboration and Program Integration: Collaborate with clinicians, therapists, staff, families, and community partners to integrate home visitation services with broader clinical and support systems.
  • Communication and Reporting: Prepare reports and communicate program activities, outcomes, progress, and needs to leadership and stakeholders.
  • Quality Improvement and Compliance: Support quality assurance, audits, monitoring, and continuous improvement while maintaining compliance with organizational, ethical, funding, and regulatory requirements.
  • Training and Professional Development: Participate in ongoing training and remain current on best practices in autism services, developmental disabilities, family-centered care, ethics, and program coordination.
