The Care Team Hospice is seeking an LPN to join our team in Mishawaka, Indiana!

Job Summary: The LPN supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.
Schedule, Hours & Territory
- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
- Mishawaka, IN
- RN Case Managers may participate in an on-call rotation with additional pay
- Some branches offer dedicated on-call staff, resulting in backup on-call coverage only
