Hospice LPN
Position Title: Hospice Licensed Practical Nurse (LPN)
FLSA Status: Non-Exempt
Job Summary:
The Hospice Licensed Practical Nurse (LPN) provides direct patient care to hospice patients under the supervision of a Registered Nurse (RN) or Physician. This role involves delivering compassionate end-of-life care, assisting with care plans, and supporting patients and their families during their final stages of life.
Duties and Responsibilities:
- Direct Patient Care (40%):
- Administer medications, treatments, and procedures as prescribed by the physician and under the direction of the RN.
- Monitor and record patient vital signs and report any changes in patient conditions to the RN or physician.
- Assist patients with activities of daily living, including bathing, dressing, and grooming.
- Care Plan Implementation (20%):
- Follow individualized care plans developed by the interdisciplinary team to meet the physical, emotional, and spiritual needs of hospice patients.
- Provide input to the RN and interdisciplinary team regarding patient care and progress.
- Ensure that all care provided is documented accurately and promptly in the patient's medical record.
- Patient and Family Support (15%):
- Provide emotional support and education to patients and their families regarding the hospice process and what to expect.
- Communicate effectively with patients and families to address concerns and ensure they feel supported.
- Assist families with the preparation for end-of-life care and decision-making.
- Interdisciplinary Team Collaboration (10%):
- Participate in interdisciplinary team meetings to discuss patient care plans and progress.
- Collaborate with other team members, including nurses, physicians, social workers, and chaplains, to provide holistic care.
- Coordinate with other healthcare providers to ensure continuity of care.
- Compliance and Training (10%)
- Follow infection control guidelines and maintain privacy rules and regulations as per HIPAA.
- Attend all meetings and complete in-services and mandatory training.
- Provide exceptional customer service and high-quality care by showing compassion and empathy towards patients and their families.
