Hospice Social Worker

Benaroya Services at Kline Galland is looking to immediately hire a Full-Time Social Worker to join our amazing Hospice and Palliative Care team. This is a meaningful opportunity to make a difference by supporting patients and families during some of life’s most important moments.

Service area: King County Eastside (Issaquah, Bellevue, Redmond, Kirkland, Woodinville, Sammamish, and nearby communities)

The Social Worker will provide a range of essential services to support the emotional, psychological, and social needs of patients and families receiving hospice and palliative care.

• Conduct comprehensive psychosocial assessments
• Develop and implement plans of care in collaboration with the interdisciplinary team
• Provide crisis intervention and therapeutic support
• Coordinate resource referrals and community services
• Support patients and families navigating end-of-life care
• Provide care across multiple settings, including:
  • Private residences
  • Adult Family Homes
  • Independent & Assisted Living communities
  • Skilled Nursing Facilities
