Hospital Liaison Lead (62281)
Aurora Mental Health & Recovery (AMHR) is seeking a Hospital Liaison Lead to provide leadership, training, and support to our Hospital Liaison Services team while also fulfilling the essential functions of a hospital liaison. This role facilitates seamless communication and coordination between AMHR clinical teams and external psychiatric inpatient facilities, including CSUs, ATUs, and psychiatric hospitals, ensuring effective discharge planning and referrals. The liaison lead will oversee staff interactions, streamline processes, and support collaborative efforts across the agency's adult intensive services division, fostering high levels of peer support and teamwork. Schedule. This position will work Monday - Friday, 8:30 am - 5:00 pm, and is eligible for a hybrid model that allows for a mix of remote and in-office work, with an expectation of being in the office two days per week and working from home three days. This schedule may be adjusted based on training, client, or community needs.
Bilingual Language Differential Pay. Employees who successfully pass a language assessment are eligible to receive bilingual differential pay. This differential is provided as a fixed amount of $3,000 per calendar year, paid on a bi‑weekly basis across 26 pay periods, and prorated based on hire date and full‑time equivalency (FTE). Eligibility and payment are subject to the language requirements of the position and the employee’s demonstrated proficiency in the language.
Salary for this role. The starting wage is based on experience and company equity. Paid bi-weekly.

Essential Functions:
- Coordinate with manager and director to develop, implement, and maintain a training process for role specific skills of all other hospital liaisons. Support the onboarding and training steps for all other hospital liaisons.
- Develop a workflow for organizing and assigning tasks and shared responsibilities amongst the hospital liaison staff. Organize and delegate hospital assignments for care coordination.
- Organize and assume primary role for ensuring completion of hospital liaison data reports including daily data reports to Colorado Access (business days), delegating when appropriate, and ensure completion of the Fort Logan fiscal year summary report.
- Manage the e-fax and email referral system ensuring that referral reports are appropriately received, distributed amongst the liaison staff, and responded to daily.
- Serve as primary point of contact representing the hospital liaison staff to external collaborating agencies.
- Provide administrative support and oversight to the hospital liaison staff regarding collaborative process steps for working with external hospital systems.
- Receiving communication from referring treatment agencies via phone, email or fax.
- Responding to referring individuals and agencies in a timely fashion, usually within the same day, to discuss referral needs.
- Providing a level of care assessment and determination for purposes of uniting clients with the treatment team that will provide the level of care needed upon discharge from the external treating facility.
- Monitoring the e-fax for clinical referral data, uploading the data to the client’s electronic record, and forwarding the clinical referral data to the assigned treatment team.
- Maintain a data tracking record with the referral information to include the day of referral and scheduled follow up appointment.
- Working with assigned hospitals to coordinate referrals of clients from acute psychiatric treatment units to other facilities for more intensive and longer-term services.
- Interacting with the Colorado Access liaisons to coordinate treatment and meet the 7 days post hospital follow up engagement goal.
- Maintain regular communication with Aurora MHC treatment teams to be fully aware of the internal referral processes used to set clients up for their needed services.
- On occasion provide education to internal and external referring individuals to assist for a smooth and collaborative referral process.
- Work closely with primary supervisor to problem solve referral issues as they occur. This needs to occur as close to possible in real time to expedite the successful referral to post-hospital and ATU services.
- Coordinate the referral process for certified clients including assisting with setting up the paperwork referral components, scheduling doctor to doctor consultations for referral of certified clients and working with the intensive services teams to coordinate the team assignment.
