IMPLEMENTATION TEAM TRAINER

Definition and Role

Highly motivated candidate who shares our vision, values, and commitment to quality healthcare. Must be able to travel 4 days per week to implement training of staff in new clinics or remediate existing staff of the positions of LPN/WMT Clinic-Home Program job descriptions and understand and able to perform the duties of all clinic staff. Must be able to perform and train in basic administrative duties such as filing, copying, and faxing documents, as well as performing data entry, provide instructions on multi-tasking in the coordination of care for patients and assist in establishing a team approach to all functions within these roles. Performs nursing procedures and direct patient care under the supervision of a MD/NP/PA.

Job Responsibilities and Duties

  • Observes, records and reports to supervising MD/NP/PA, the general physical and mental conditions of the patient, as well as changes in the patient’s condition.
  • Administers prescribed medications and treatments, in accordance with the plan of treatment, as permitted by State and local regulations.
  • Assists the MD/NP/PA in performing specialized procedures.
  • Prepares equipment and materials for treatment and adheres to aseptic and/or sterile technique.
  • Assists the patient with activities of daily living and in learning appropriate self-care techniques, including wound care dressing changes.
  • May occasionally be required to assist with Wound Medicine visits in the home setting through our Home to Heal Program
  • Implements the nursing component of the patient’s plan of care.
  • Assists the MD/NP/PA in teaching, supervising, and counseling the patient and family members, regarding the nursing care needs of the patient. (Follow-up on the plan developed by the MD/NP/PA).
  • Prepare progress notes and clinical notes and incorporate them into the electronic medical record as indicated.
  • Notifies MD/NP/PA or charge nurse of any problems identified in patient’s living environment, whether it is acute hospital, LTAC, Nursing home, or patient's home.
  • Attends case conferences/interdisciplinary team meetings to discuss patient’s problems, treatment plans, patient progress and patient’s overall health status.
  • May perform routine venipuncture if written documentation of competency is in personnel record. Must demonstrate competency in performing the procedure.
  • Performs wound care and dressing changes as directed by the MD/NP/PA. Including competency on advanced wound care dressings. i.e.: TCC, dermal skin substitutes, compression wraps, etc.
  • May receive orders from the MD/NP/PA and follow those orders, that are within the realm of practice for an LPN, and within the standards of practice for the facility in which he/she is working in.
  • Responsible for “all other duties assigned”.
  • Completes any necessary paperwork and turns in daily as indicated. Including documentation within the company’s EHR.
  • Perform photo documentation and picture taking of wounds.
  • Travel may be required to various clinic locations and home program visits.
  • Nails are required to be trimmed and cleaned.
  • Acrylic nails are prohibited.
  • Adheres to dress code and wears identification while on duty.

A Wound Medicine Technician Provides for the delivery of patient care, works collaboratively and under the direct supervision of a licensed nurse/provider; communicates effectively with coworkers, patients, family, and visitors; provides a safe environment for patients; assists patients in obtaining optimal functioning considering their individual capabilities; performs all functions as delegated by the lead nurse; and keeps nursing personnel informed and adheres to the policies and procedures of MedCentris.

Job Responsibilities and Duties

  • Dressing removal, wound cleansing, taking wound photos, measurements & vital signs; set up and clean up patient treatment areas; instrument cleaning, set up for special procedures & sterilization of instruments
  • Dressing application under direction of licensed personnel
  • Competency completion for multiple skills as appropriate for scope of practice
  • Inventory control and ordering as directed by Clinical Coordinator or Program Director.
  • In the field techs are responsible for restocking inventory as directed by their Provider or Program Director
  • Able to perform tasks involving physical activity, which may include lifting and carrying at least 50lbs, transferring and repositioning patients, and extensive bending and standing.
  • Assists with completion of patient testing and treatment procedures
  • Assists with processing cultures, labs, biopsies, and various ancillary orders
  • Performs required documentation and communicates in a competent and effective manner to meet the needs of the patients, families, and other staff members
  • Must complete charting in a timely manner.
  • Charting is to be completed at the facility or immediately after patient care/rounds.
  • In the field techs are to assist in obtaining all consents and adding them to the appropriate “Spark” folder
  • Assists with appointment confirmation, patient status reports, patient satisfaction reporting, patient disposition management and wound status updates
  • Able to perform multiple administrative tasks and has adequate computer skills
  • Able to assist with the safe transport of patients by wheelchair or stretcher
  • Utilizes time and resources effectively to promote quality and cost outcomes
  • In the field techs are to complete time tracking sheet and have them signed by your provider daily once completed
  • In the field techs are to turn in completed time tracking sheet to supervisor by the end of each week
  • In the field techs are to complete mileage form and turn in to your supervisor weekly
  • In the field techs are to turn in the following weeks scheduled times and locations to supervisor by noon every Friday
  • Works collaboratively under the direct supervision of a licensed nurse
  • Attends and participates in meetings upon request, and continuing education.
  • Ability to communicate, work effectively and interact well with providers, colleagues and other healthcare professionals and always maintain employee and patient confidentiality in written, verbal, and electronic information.
  • Assists Provider and professional staff with equipment and supply set up and reports any defect or compromise in a timely manner

  • Faxing, DME orders, calls & all administrative support tasks as directed by Charge Nurse
  • Appropriate & timely reporting, to Charge Nurse or Manager, of any issue, concern, suspicion or communication that presents a compromise to patient safety, wound outcomes or client satisfaction
  • EHR data entry under direction of Charge Nurse or Provider
  • Supply procurement for offsite OP settings
  • Report office supply or medical supply needs
  • Keep treatment rooms stocked
  • Keep supply closet clean & organized
  • Frequently checking company email and Spark during business hours
  • Monitor time & attendance, refraining from OT w/out prior approval
  • Process & autoclave surgical instruments
  • Assistance with EHR patient/wound status updates (patients with extensive absences, deceased, discharged, transferred, etc.; needs to be done q 2 weeks at a minimum to ensure more accurate statistics; always check with receptionist or Clinical Coordinator on the status of this task.
  • All other duties assigned/requested by your direct supervisor.
  • Required to work in neighboring clinics within a 60-mile radius as well as the Home to Heal driving requirements.

A Clinical Manager manages the day-to-day operations of Wound Medicine Clinic. Supervises clinic staff to include nurses and techs. Performs nursing procedures and direct patient care under the supervision of a MD/NP/PA. Must have a good working knowledge of basic office equipment and good phone and computer skills. They are expected to multitask and adequately solve problems while providing quality service and maintaining a positive relationship with the executive team, nurse practitioners, physicians, healthcare providers, and administrative staff. It is also important that the Clinic Manager follows all rules related to patient and employee confidentiality.

  • Supervision of clinic staff to include scheduling, monitoring productivity, controlling overtime, ensuring appropriate workflows in clinic to facilitate timely provision of care to patients in the clinic.
  • Management of supply inventory, ordering of supplies, maintaining appropriate par levels
  • Facilitate communication with other team members to quickly resolve any patient or clinic issues
  • Ensures appropriate approvals from billing related to specialty items required for patient care
