In Home Family Support Specialist

Glenwood is seeking a Family Support Specialist to join our team. This position plays a critical role in ensuring high-quality, family centered services through strong coordination, communication, and data management. provide support, guidance, and resource coordination to children and families participating in at home, autism-related services. In this role, you will be responsible for assisting families throughout the referral, intake, assessment, and service navigation process while helping connect them to community resources and supports. The Family Support Specialist will work collaboratively with interdisciplinary team members to ensure families receive timely access to services and individualized support.

  • Family Engagement and Support: Build and maintain supportive relationships with families by providing guidance, education, and ongoing assistance throughout the service delivery process.
  • Intake and Referral Coordination: Conduct outreach to referred families, provide program information, and coordinate intake, enrollment, and onboarding into Autism Home Visitation services.
  • Home-Based Services: Conduct home visits to provide direct support, complete assessments, coordinate services, and identify family strengths and needs.

  • Family Resource Development: Develop, maintain, and distribute family resource materials, referral information, and community resource guides.
  • Team Collaboration: Collaborate with Autism Home Visitation staff and interdisciplinary team members to support family needs and ensure effective delivery service.
  • Meeting Participation: Attend and actively participate in team meetings, case consultations, training, and other organizational meetings.
  • Program Support: Provide operational and programmatic support to Autism Clinic services to ensure efficient delivery service.
  • Documentation and Compliance: Maintain accurate, timely, and compliant documentation in accordance with agency policies, funding requirements, and regulatory standards.
