The Palliative Care Manager is responsible for facilitating goals of care discussions, patient, family and interdisciplinary meetings and services for patients at the end-of-life. This role ensures effective utilization and monitoring of healthcare resources while managing interdisciplinary collaboration to achieve optimal clinical and resource outcomes. The Palliative Care Manager works closely with the Palliative Care team and the Manager of Palliative Care to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize patient outcomes and ensure safe, high-quality care.
Principal Accountabilities:
- Manage, coordinate, and facilitate Palliative patient care through comprehensive patient and family assessments and multidisciplinary collaboration.
- Formulate, coordinate, and implement individualized plans of care and discharge plans to ensure safe, timely, and effective transitions.
- High focus upon Goals of Care discussions with patients, their families, designated health care decision-makers or guardians.
- Perform daily chart reviews of Palliative care patients, coordinate discharge planning activities, ensuring thorough and accurate clinical documentation in the electronic medical record.
- Participate in interdisciplinary Palliative Care patient care rounds, identifying and resolving quality of care issues, barriers to care, and delays to discharge home or continued stay on Hospice status.
- Serve as a representative of the department and participate in hospital meetings as needed.
- Perform other related duties as assigned.
Required
- Bachelor of Science in Nursing (BSN)
- Current, valid New Jersey Registered Nurse license
Preferred
3 – 5 years of RN Case Manager palliative care experience in an acute care, interdisciplinary team environment
