Lead Case Manager

JOB PURPOSE:

The Lead Case Manager will provide oversight, leadership, and direct support in the delivery of case management services to residents of Cady Lofts, a Permanent Supportive Housing (PSH) community serving individuals with histories of chronic homelessness, mental health challenges, substance use, and complex medical conditions. This role combines direct case management, and team leadership to ensure high-quality, person-centered services aligned with trauma-informed care principles.
