Under the general direction of the Department Manager, the Lead Case Manager monitors and coordinates Case Management services, and assists the Department Manager with monitoring, orientation, training, and problem solving of the case managers. In addition, the Lead Case Manager also coordinates the care and services of patients across the continuum of their hospitalization. The lead case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient’s treatment and discharge needs, which would achieve optimal clinical and resource outcomes.
Education/Training: Masters Degree in social work or RN with BSN.
Experience: Minimum of five years of progressive experience working in a, medical settings and at least 2 years supervisory experience.
Licensure, Registration or Certification: Current New York State Licensure as a Registered Professional Nurse, or New York State Licensure to practice Social Work in the State of New York. Certification in Case Management required by 3/31/2013. PRI certification preferred.
