What You'll Do

Direct Clinical Services

  • Conduct comprehensive biopsychosocial assessments to evaluate client needs, strengths, and risk factors
  • Provide individual, family, and group therapy using evidence-based modalities (CBT, DBT, trauma-informed care, motivational interviewing, etc.)
  • Develop and implement individualized treatment plans in collaboration with clients and care team members
  • Monitor and document client progress, adjusting treatment plans as clinically indicated
  • Provide crisis intervention, safety planning, and stabilization services as needed

Care Coordination & Advocacy

  • Collaborate with physicians, nurses, case managers, and community health workers to ensure integrated, whole-person care
  • Connect clients with community resources including housing assistance, food security programs, transportation, legal aid, and benefits enrollment
  • Facilitate warm handoffs and referrals to specialty mental health, substance use, or medical services

Documentation & Compliance

  • Complete timely and accurate clinical documentation in the electronic health record (EHR) in accordance with organizational and regulatory standards
  • Maintain confidentiality and adhere to HIPAA and applicable state regulations
  • Participate in quality improvement initiatives, case reviews, and outcome measurement efforts

Community Engagement

  • Build trusting relationships with clients, families, and community partners across diverse cultural backgrounds
  • Participate in educational workshops
  • Contribute to a welcoming, culturally humble, and trauma-informed organizational environment

What We’re Looking For

  • Active Licensed Clinical Social Worker (LCSW) license
  • 3+ years of post-licensure clinical experience (community mental health, integrated care, or similar settings preferred)
  • Comfort providing therapy independently within a supported care model
  • Strong clinical judgment in assessment, diagnosis, and treatment planning
  • Clear, professional communication and collaboration with care teams
  • Comfort using telehealth platforms and clinical documentation tools
  • Commitment to full-time work and continuity of care
