  Responsibilities:<\/span><\/span><\/b>
  <\/p>

  • Provide patient\n care according to client\u2019s care plan and needs<\/span><\/span>
    <\/li>
  • Perform wound\n care treatments and assist clients with medication management<\/span><\/span>
    <\/li>
  • Teach the\n client and/or family members about care and support skills<\/span><\/span>
    <\/li>
  • Communicate\n regularly with RN Case Managers, Supervisors, and the Care Team<\/span><\/span>
    <\/li><\/ul>

    Qualifications:<\/span><\/span><\/b>
    <\/p>

    • Licensed as a\n Practical Nurse / LPN in the state of Minnesota<\/span><\/span>
      <\/li>
    • Driving is\n required; must have a reliable vehicle with current insurance and a valid\n Driver's License<\/span><\/span>
      <\/li>
    • Demonstrated\n ability to work autonomously<\/span><\/span>
      <\/li>
    • Prior Nursing\n experience in a TCU, Hospice, or Home Health setting<\/span><\/span>
      <\/li>
    • Experience with\n Electronic Medical Records (EMR) software is preferred<\/span><\/span>
      <\/li><\/ul>
