Medical Assistant Patient Care Coordinator

This role is centered on delivering consistent, patient-focused care coordination and health coaching for individuals managing chronic conditions. You will serve as a key point of contact for a large patient panel, ensuring regular outreach, continuity of care, and support in following care plans. The position combines clinical knowledge, communication skills, and strong organizational ability to help close gaps in care and improve patient outcomes. You will collaborate closely with clinical teams, providers, and caregivers to identify needs, address barriers, and ensure patients receive appropriate support. A major part of the role involves high-volume daily patient communication, documentation accuracy, and adherence to HIPAA-compliant practices. This is a mission-driven remote role where empathy, consistency, and responsiveness directly impact patient well-being.
Accountabilities:
- Manage a panel of 400–450 patients and conduct monthly outreach to ensure continuity of care and adherence to care plans.
- Perform 50–60 outbound patient calls daily, providing health coaching and support aligned with individual patient needs.
- Collaborate with providers, clinical teams, and caregivers to identify clinical and social care needs and coordinate appropriate interventions.
- Support care coordination programs such as Chronic Care Management, Behavioral Health Integration, and Remote Physiological Monitoring.
- Advocate for patients by actively listening, addressing concerns, and communicating effectively with the broader care team.
- Maintain accurate, real-time documentation of all patient interactions using designated care coordination platforms.
- Ensure all communications and records comply with HIPAA regulations and internal quality standards.
- Must hold a nationally recognized Medical Assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent MA-C) OR be a Certified Pharmacy Technician (CPhT).
- Minimum 1 year of clinical experience in a healthcare setting such as primary care, hospital, or specialty practice.
- Experience working with Electronic Medical Records (EMR) systems and strong overall computer proficiency.
- Ability to navigate multiple healthcare and communication platforms efficiently in a fast-paced environment.
- Strong communication skills with the ability to build rapport, educate patients, and collaborate with care teams.
- Highly organized, detail-oriented, and comfortable managing high patient volumes and outreach targets.
- Must be authorized to work in the United States and maintain a HIPAA-compliant home workspace with reliable internet.
- Preferred: experience in care coordination, chronic care management, RPM programs, or behavioral health integration; bilingual English/Spanish is a plus.
- Competitive hourly compensation ranging from $17.00 to $20.00 based on experience.
