As a Medical Management Team Lead, you would be responsible for running our exciting population health outcomes. To be a successful Medical Management Team Lead, you’ll need to be able to communicate the protocol set forth by our Population Health Nurse and Team Pharmacist. We need someone with a great desire to work hard managing on a granular level the ins and outs of our Population Health, being hungry for growth and opportunities, always eager to jump in and embrace the challenge.

You will succeed if you can work effectively with our Population Health Nurse and Team Pharmacist, be detail oriented, and be able to work on the granular level to achieve positive health outcomes for groups and their members.

THE POSITION

Medical Management Team Lead responsibilities

  • The applicant will work with population health third parties to develop actionable deliverables to customer base
  • Monitor data from various TPAs based on protocols set by our Population Health Nurse and Team Pharmacist.
  • Handle day to day interactions with customers who use third party disease management vendors

·Manage Population Health

  • Enforce the protocols set by Population Health Nurse and Team Pharmacist on a granular level

Requirements

A successful candidate will have:

·4 Year Degree

  • CNA Background (Nursing home equivalent)

Preferred qualifications

  • Background interacting with medical carriers, TPAs, and PBMs
  • Familiarity with medical and pharmacy claims data
  • Experience with corporate wellness/disease management programs
