The Member Care Scheduler will serve Home Care and Community Supported Living Members that are frail elders, persons with physical disabilities and/or persons with intellectual/developmental disabilities. The goal for you and your Team is to ensure that these Members receive the services that are authorized for them, which promote their overall well-being. Also, the goal for you and your Team is to ensure that Clarity Care’s promise to provide high quality care is being met.

Key Responsibilities:

  • Understand member assessments, needs and outcomes
  • Manage scheduling software for quality care
  • Create biweekly schedules matching authorized service hours
  • Ensure members always receive attended services
  • Provide professional, responsive customer communication
  • Update systems, schedules as conditions change
  • Maintain on-call coverage rotation requirements

Education/Experience:

  1. A Bachelor’s degree in Human Services, Psychology or Supply Chain Management. An Associate’s Degree will be considered. Or, related field with two (2) years’ experience working with elders or persons with disabilities.
  2. Possess advanced computer and telecommunication skills, with proficiency in Microsoft Office Suite.
  3. Excellent organizational, time management, and critical thinking skills.
  4. An energetic, outgoing, detail-oriented person who demonstrates strong written and verbal communication skills.
  5. Ability to be resourceful and proactive when issues arise.
  6. Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
  7. Must pass periodic WI Caregiver Background Checks including Department of Justice Criminal Record Checks, and/or that of previous states of residence
