Schedule: Exempt (salary), min 40 hours per week | Monday - Friday | 8:30am - 5pm | Occasional work hours outside regular business hours as needed

What You’ll Do:
The Memory Care Program Manager is responsible for the development, implementation, and oversight of Teepa Snow’s Positive Approach™ model, designed to enhance the quality of life for residents with memory impairments. The Program Manager will lead a team of caregivers, provide ongoing team member training, and work closely with families to ensure a collaborative approach to care.
- Design, implement, manage, and support Positive Approach™ Model to next level of designation.
- Develop and oversee individualized care plans and therapeutic programs tailored to each person's unique needs and goals.
- Develop and oversee daily activity schedules.
- Establish and maintain relationships with local Alzheimer’s and dementia organizations.
- Works in collaboration with the interdisciplinary team on resident care plans, including participating in family communications and care conferences.
- Conduct regular assessments of residents to monitor cognitive and functional status, adjusting care plans as needed.
- Implement a formal training program utilizing the Positive Approach™ model for all team members, residents and families.
- Provide ongoing education and training on dementia care.
- Provide education and marketing for Positive Approach™ Model.
- Provide monthly Alzheimer education for the community.
- Serve as a point of contact for families who are dealing with dementia and other cognitive impairments by providing guidance and education.
- Conduct regular audits of care plans, medication management, and safety protocols to maintain high standards of care.

- Bachelor’s degree in Gerontology, Social Work, or a related field (Master’s preferred).
- Minimum of two (2) years of experience in memory care, dementia care, or a related field.
- Minimum of three (3) years of experience working with the elder population.
- Working knowledge and proficiency of electronic medical records, Point Click Care software preferred
- Positive Approach to Care (PAC) certification, or ability to obtain within six (6) months of hire.
- Certified Dementia Practitioner (CDP) or similar certification preferred.
- Strong knowledge of all forms of dementia, including current best practices for care.
- Position requires the ability to lift and move objects weighting up to 35 pounds without assistance.
