CHI St. Alexius Health Dickinson, part of CommonSpirit Heath, a national nonprofit health system based in Englewood, Colorado. The faith-based system operates in 18 states and includes 103 hospitals. Additional services offered within the system are: long-term care, assisted and residential living communities, community health services organizations, home health agencies, and numerous outpatient facilities.
Job Summary and Responsibilities
As our RN Clinic Navigator at CHI St. Alexius Health Dickinson, you will help patients and their families seamlessly navigate their healthcare journey so they can access comprehensive care and achieve optimal health outcomes.
Every day you will coordinate care, educate patients, and connect them with vital resources and support. You will be expected to facilitate interactions between patients and their clinical care team, empower patients with information about their conditions and treatments, and collaborate with providers to implement care standards and secure appropriate services.
To be successful in this role, you must possess strong collaborative and communication skills, a compassionate approach to patient care, and the ability to comprehensively assess patient needs and connect them with vital resources.
- Care Coordination: Facilitate interactions between patients and their clinical care team, ensuring a smooth and coordinated experience throughout the continuum of care.
- Patient Education: Empower patients, families, and caregivers by educating them about diseases, treatments, side effects, and how to access reliable information.
- Resource & Support: Collaborate with providers to implement care standards, and identify/secure appropriate services. You'll connect patients with crucial resources for psychosocial distress or tangible barriers to care (e.g., finances, housing, transportation), referring to social workers or counselors as needed.
- Clinical Access: Provide resources for specialized treatments or testing not available locally, and promote awareness of appropriate clinical trials.Patient Empowerment: Encourage patients, families, and caregivers to take an active role in managing their care, within the scope of their individual circumstances.
Job Requirements
Required
- Graduate of an accredited school of nursing
- Active, unencumbered Registered Nurse (RN) license from North Dakota
- Basic Life Support (BLS) or Advanced Care Life Support (ACLS)
Preferred:
- Bachelor of Science in Nursing
- 1 year related experience in a clinic, specialty clinic or acute hospital setting
