Nutritional Support Coordinator

This remote healthcare role focuses on delivering consistent nutritional guidance and care coordination to patients managing chronic conditions. You will act as a key point of contact, supporting patients through regular outreach, education, and motivational coaching. The role involves high-volume daily communication, requiring strong organization and empathy in every interaction. You’ll simplify nutrition concepts, reinforce provider care plans, and help patients stay engaged in their health journey. Working within a structured clinical framework, you’ll collaborate closely with care teams to ensure timely escalation of concerns. This position is fully remote but requires a dedicated, HIPAA-compliant home workspace. It’s ideal for someone who thrives in fast-paced, patient-centered environments.
Accountabilities:
- Manage a panel of 300–350 patients, ensuring consistent monthly outreach and nutritional support aligned with care plans.
- Conduct 50–60+ daily patient calls, delivering nutrition education, coaching, and adherence reinforcement tailored to individual conditions.
- Educate patients on dietary changes, weight management strategies, and chronic disease nutrition support in an accessible and compassionate manner.
- Collaborate with clinical staff, providers, and caregivers to coordinate care and escalate changes in patient condition when needed.
- Maintain accurate, real-time documentation of all patient interactions within designated care coordination systems.
- Ensure compliance with HIPAA regulations and uphold quality standards across all communications and documentation.
- Minimum 2+ years of experience in a healthcare-related role such as care coordination, health coaching, or medical assisting (CMA, RMA, CCMA, NCMA, CMAC or equivalent certification), or as an LPN/LVN with an active multi-state compact license.
- At least 2+ years of experience in clinical nutrition counseling, dietary support, or chronic care management.
- Strong proficiency in EMR systems and general computer navigation.
- Proven ability to explain complex nutritional or clinical concepts in simple, patient-friendly language.
- Experience working with high patient volumes and structured outreach targets.
- Must have a HIPAA-compliant home office setup and reliable internet (minimum 50 Mbps download / 5 Mbps upload).
- Must be legally authorized to work in the United States.
- Preferred: NDTR, Certified Nutritionist, or Certified Health Coach credentials; bilingual English/Spanish abilities; experience in telehealth or chronic disease programs.
