About the role:Reporting to Manager of Oncology Pharmacy, the successful candidate will be responsible for assessing, planning implementation and evaluating patient care and practice activities in a multidisciplinary environment. The Pharmacist is responsible for order processing, dispensing and formulation, evaluation of patient therapy, provision of medication, patient education/counselling.
What you'll be doing:
Demonstrates the ability to provide drug therapy monitoring and pharmaceutical care.
Demonstrates an understanding of funding related to oncology drugs in Ontario
Demonstrates an understanding of sterile compounding procedures, safe handling of hazardous drugs, and the NAPRA standards.
Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
Evidence of sound organizational and prioritization skills. Ability to work well in stressful situations
Demonstrates good communication skills including strong documentation skills
Demonstrates ability to work well with patients, families, all levels of staff and the community
Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
Other duties as assigned.
What you bring:
Completion of university degree (Bachelors of Science Degree in pharmacy or PharmD) or equivalent required
Active registration in good standing with the Ontario College of Pharmacists (Part A) required
Membership in the Canadian Society of Hospital Pharmacists an asset.
Completion of a hospital residency or previous hospital or cancer centre dispensing and clinical experience required
Oncology experience with chemotherapy preparation and clinical monitoring is preferred
Demonstrated excellence in communication, both written and verbal
Demonstrated analytical and problem assessment skills
Demonstrated commitment to customer service and teamwork
Demonstrated individual leadership skills
Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
