Title: Outreach Worker/Group Facilitator

Unit: Admissions/Alternative to Incarceration

Reports to: Director of Outreach / Director of Alternatives to Incarceration

Status: Full, Time; Regular; Non-Exempt

Location: Five boroughs & Long Island City

Days/Hours: Monday to Friday, 9:00am to 5:00pm, some evenings 11:30am to 7:30pm, subject to change depending on program needs.

Salary Range: $24.73 to $27.48 per hour (approximately $45,000-$50,000 annually)

Position Summary:

Responsible for successfully conducting outreach (including disseminating outreach materials) at a variety of community-based and government agencies throughout the five boroughs AND will facilitate ATI evidence-based workshops and groups.

Essential Duties and Responsibilities:

  • Present on Fortune’s wrap-around services at a variety of community-based and government agencies and at forums including health fairs, community events, farmer’s markets, etc.
  • Handout printed materials to individuals and groups at presentations.
  • Liaise with referral sources, promoting Fortune services.
  • Accept and engage with new client referrals.
  • Connect incoming clients with Fortune’s wrap-around services, including linkages to benefits such as SNAP.
  • Identify SNAP eligible individuals and households.
  • Attend monthly SNAP taskforce meetings or any training related to SNAP eligibility.
  • Follow a weekly field schedule and provide daily and accurate reports of areas covered.
  • Find/research outside organizations to network within order to schedule presentations.
  • Able to work effectively both independently and collaboratively.
  • Provide real time actionable recommendations on how to maximize outcomes for the program.

    • Seeking Safety
    • Anger Management
    • Thinking for a Change
    • Interactive Journaling
    • Parenting for Dads
  • Ensure groups start on time and run as scheduled
  • Be prepared with needed tools to increase client participation
  • Assist clients in navigating their scheduled group attendance
  • Assist clients in adhering to group rules of participation, such as no lateness, cell phone use, eating, drinking, foul language, disrespect of other group members.
  • Participate in case conferences for optimum collaboration among departments and programs
  • Communicate with other staff on any relevant behavioral concerns regarding a client that may arise during their group participation
  • Must be team player and work colleagues to address programmatic challenges as they arise
  • Enter timely and accurate daily group attendance in the database systems
  • Enter daily group notes into the database systems
  • Maintain electronic and hard copy filing system
  • Attend and participate in scheduled staff activities, meetings and trainings
  • Engage in his/her own ongoing professional development.
  • Perform other duties or tasks as required.
