Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.

As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.

The PACE Day Center Manager oversees the day-to-day operations of the PACE Day Center and coordinates non-clinical services delivered to participants. This role is responsible for managing participant flow, day center operations, non-clinical staff, environment-of-care readiness, and regulatory compliance to ensure safe, efficient, and high-quality service delivery. Working under the direction of the Center Director, the Center Manager supports interdisciplinary workflows, enhances the participant experience, and ensures facility safety and operational consistency. The role also includes oversight of CNA, PCA, and Recreation (Activities) services. This position is fully onsite. Schedule: Monday-Friday, 8am-5pm.

Responsibilities

  • Provides leadership to all aspects of the day center operations, including participant flow, personal care support, and Activities programming.
  • Maintains competency in Interdisciplinary Team (IDT) processes and facilitates IDT meetings as directed, ensuring effective coordination of care and operational alignment.
  • In coordination with the Center Director, supports utilization management activities
  • Actively participates and leads IDT meetings, fulfilling assigned responsibilities.
  • Reviews staffing levels, enrollment and center capacity to address and resolve operational issues
  • Provides oversight of contract employees and any service providers operating in the day center and collaborates with the Quality and Compliance Officer for contractor incident reporting and corrective action plans as needed
  • Supports IDT workflows to ensure timely development, documentation, and communication of participant care plans. Ensures documentation of team meetings is maintained and that all care plan changes are accurately recorded in individual care plans.
  • Ensures all required regulatory notifications are completed when care plans change.
  • Coordinates orientation, in-service programs, and ongoing staff development to meet regulatory and performance requirements.
  • Serves as a liaison between NHC PACE and other community service providers
  • Participates in the NHC PACE management team and serves as liaison between administration and staff
  • Works with Quality and Compliance on participant enrollment and disenrollment processes in alignment with regulatory requirements.
  • Reviews participant rights issues, grievances, and appeals with the Center Director and Quality and Compliance, ensuring proper documentation, investigation, and resolution.
  • In coordination with the Center Director and Quality and Compliance Officer, communicates with contract providers and resolves administrative, rights-related, or quality-of-service concerns.
  • Participates in NHC PACE Quality Improvement (QI) activities and relevant sub-committees
  • Supports implementation of day center quality improvement initiatives related to safety, participant experience, and operational performance.

  • Ensure compliance with policies regarding physical and chemical restraints.
  • Ensure compliance with federal, state, and local regulatory and licensing standards
  • Maintains continuous audit readiness for all day center operations.
  • Participates in the development and delivery of staff education programs and may facilitate meetings as needed.
  • Oversee documentation processes and ensure compliance with HIPAA and internal documentation standards.
  • Oversees maintenance, cleanliness, and repair of day center equipment, common areas, and environmental systems in collaboration with Facilities.
  • Monitors participant and staff safety; conducts environmental rounds; investigates and reports accidents promptly.
  • Ensures infection control and emergency preparedness protocols are implemented and maintained within the center Participates in program and policy development and refinement as part of the management team
  • Participates in program and policy refinement activities as part of the management team
  • Supports budget monitoring and ensures day center operations are conducted within approved fiscal targets.
  • Consistently meets or exceeds NHC PACE’s targets for productivity and customer service expectations
  • Ensures standardization of operations across centers; identifies and reports variances with rationale and escalates for alignment.
  • Leads, coaches, and develops assigned staff, including hiring, onboarding, performance management, and disciplinary actions in coordination with Human Resources
  • Provides directions to site-level staff and supervisors to ensure alignment with established workflows, performance expectations, and organizational priorities
  • Conducts regular team meetings to promote communication, accountability, and alignment
  • Evaluates staff performance and supports ongoing development through coaching, feedback, and competency-based assessments
  • Ensures appropriate staffing coverage and operational support across assigned functions
  • Promotes adherence to organizational mission, values, and service standards
  • Provides coverage for staffing gaps, including LOA, PTO, and high-volume periods, by stepping in to support operations as needed
  • Promotes and protects participant rights, including dignity, independence, privacy, and self-determination.
  • Represents NHC PACE in community activities that promote program visibility and strengthen community relationships.
  • Collaborate effectively with peers, contractors, and interdisciplinary partners to achieve organizational goals.
  • Comply with all policies and procedures of NHC PACE
  • Demonstrates proficiency in delivering age-specific care
  • Consistently meets or exceeds NHC PACE quality assessment and performance improvement targets
  • Travels between PACE centers, community locations, and external provider sites as needed to support operations, meetings, participant services, and organizational initiatives.
  • Performs other duties as assigned.

Site Specific Responsibilities (Murrieta)

  • Oversees day-to-day operations of Alternative Care Settings (ACS) to ensure timely, accurate, and compliant service delivery.
  • Ensures alignment with participant care plans and regulatory requirements.
  • Coordinates ACS activities with Enrollment, Clinic, Day Center, and IDT teams.
  • Ensures workflows are standardized across centers; identifies variances and escalates with rationale.
  • Monitors timeliness, quality, and completion of ACS services to ensure compliance with organizational and regulatory standards.
  • Identifies and escalate operational, safety, and compliance risks to support timely resolution.

Qualifications

Education/Experience

  • Bachelor’s degree in rehabilitative therapy, nursing, social work, recreational therapy, or related field is required, experience in lieu of degree is acceptable (must be experience in one of the specialties above)
  • Master's degree in rehabilitative therapy, nursing, social work, recreational therapy, or related field preferred
  • Current Basic Life Support (BLS) certification required upon hire and must be maintained as a condition of employment. These courses must follow AHA guidelines- may be completed through approved online providers such as ProMed or other equivalent programs that meet recognized BLS standards.
    • In addition, current CPR and First Aid certification are required for all PACE employees in accordance with CMS and DHCS requirements. First Aid training will be provided at onboarding and must be renewed annually.
  • Three years of experience working with elderly population in a healthcare setting required
  • Two years of supervisory experience required
  • Valid Drivers License required and maintained as a condition of employment
  • Active auto insurance required and maintained as a condition of employment

Additional Qualifications(Knowledge, Skills and Abilities)

  • Knowledge of health care delivery systems, regulations, quality improvement and medical and non-medical needs of the elderly is preferred
  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Pay range: $95,500-$141,300 annually, depending on experience.
