Palliative Care RN

GENERAL SUMMARY:

The Palliative Care Nurse provides Palliative Care Services to patients in the clinic, hospital, and community settings. The Nurse must be able to think and work independently, creatively, and productively with an interdisciplinary team of healthcare workers. This work will be complex – both broadly conceptual and, at times, tedious and detailed – requiring a mature, holistic approach to be successful. The Nurse is primarily responsible for coordinating and expediting care for palliative care patients; effectively communicating with patients, providers, and community support organizations to coordinate and facilitate a comprehensive plan of care; and facilitating a shared goal model within and across settings to achieve coordinated, high-quality care that is patient/family centered. The Nurse demonstrates excellence in customer service during interactions with patients, families, and other customers.
