Patient Care Coordinator

Job Description:

The Patient Care Coordinator will coordinate all aspects of the patients care allowing for seamless communication between the patients and all disciplines assigned to the case.

What we are looking for:

  • High School Graduate or GED

  • Two years recent work experience, preferably in home care

  • Strong verbal skills and written communication skills.

  • Good organizational skills.

  • Good customer service skills

  • Bilingual preferred
