HIA Hospice (formerly Hospice of the Red River Valley) is looking for a motivated, independent, and responsible Registered Nurse (RN) to join our growing clinical team and service area. This position offers an exciting opportunity to join a well-known and 40+ year old hospice organization dedicated to the mission of "Providing comfort and care through life's journey". The ideal candidate should have experience in hospice and/or home health with proven success as a leader and mentor.

Position Summary:

The Patient Care Manager (PCM) manages the daily operations of a hospice clinical team to ensure optimum quality patient and family care. Effectively coordinates clinical operations in collaboration with other clinical managers, senior managers, directors, and all inter-disciplinary team members; drawing on resources to achieve common goals. Promotes teamwork as a way to generate positive results. This position is full time and will be based out of our Detroit Lakes office and serve the surrounding area.

Pay Range: Starting at $38.39/hour (based on experience)

Key Responsibilities:

  • Coordinates the daily operations of the clinical team; scheduling, staffing, triaging calls and team needs, IDG meetings, pharmacy, DME, supply coordination, and customer service.
  • Ensures hospice team is providing and documenting high quality, goal directed care, accurately and in a timely manner, adhering to the standards of practice and regulatory and licensure requirements, along with the nursing clinical guidelines.
  • Facilities the IDG meetings, assists in problem solving and solution finding, and ensures completion of appropriate documentation.
  • Participates in performance improvement initiatives and promotes quality assurance.
  • Develops staff through training, mentoring, and coaching.
  • Sets clear expectations, ensures competency through the education and orientation of the team in collaboration with the Director of Clinical Operations.
