Responsible for daily patient safety improvement for assigned ministry(s), medical group practices, and/or business units. Works with leaders, clinicians, and other staff in the development and implementation of strategies to identify and reduce risks, improve safety, and meet regulatory requirements. Ensures implementation of an integrated patient safety program to position the organization as a national leader in patient safety.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Responsible for integrating patient safety into the organizational structure and culture and daily routine by coaching clinical and administrative leaders regarding methods and error prevention strategy, and by disseminating lessons learned across the organization. Leads patient safety improvement initiatives, such as Regional and System patient safety committees, workgroups, and meetings.
- Investigates patient safety issues using event analysis methodology to conduct critical incident reviews (ACA, RCA, FMEA). Coordinates the FMEA (Failure Mode Effects Analysis) process and other patient safety tools such as patient risk assessments. Leads the annual analysis of the patient safety plan.
- Monitors the progress of safety improvement changes and manages the event reporting process. Works with departments to mitigate immediate risks to patients due to human or system errors through various cause analysis approaches.
- Prepares and presents reports for leadership, safety committees, and regulatory agencies. Demonstrates mastery of facilitation skills, data analysis, and statistical process control to improve corporate quality goals and clinical outcomes.
- Evaluates trends to identify system opportunities that lead to patient harm and leads efforts for systems improvement to prevent such harms. Provides input and guidance to leadership in the management of these events, offering feedback to leadership to drive continuous improvement.
- Collaborates, coaches, and partners with senior leadership in response to multiple safety projects, including Culture of Safety survey response and action planning, and initiatives promoting a Just Culture and High Reliability organization.
- Rounds routinely in all clinical care departments to assess and address risk reduction and patient safety activities/concerns, following up with department managers and leadership to ensure risk mitigation.
- Leads, prepares, and presents information to all levels of leadership as a subject matter expert and consultant to support implementation of patient safety programs and High Reliability initiatives.
- Leads, plans, prepares, and coordinates presentations and educational opportunities for new employees and providers regarding patient safety strategies. Facilitates and supports implementation of patient safety programs, Just Culture and High Reliability initiatives.
- Manages and supports multiple quality projects related to patient safety simultaneously.
- Performs other duties as assigned.
EDUCATION
Bachelor's degree or equivalent years of experience and education
EXPERIENCE
Three years' experience
PHYSICAL REQUIREMENTS
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
Frequent keyboard use/data entry.
Occasional bending, stooping, kneeling, squatting, twisting and gripping.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
Rare climbing.
