Role Scope:

The Physical Therapist Assistant (PTA) provides high-quality rehabilitative support for participants enrolled in the Habitat Health PACE program (Program of AllInclusive Care for the Elderly). Working under the supervision of a licensed Physical Therapist, the PTA delivers therapeutic interventions that promote mobility, safety, independence, and overall quality of life. This perdiem role offers flexible scheduling while supporting participant needs across clinic, home, and daycenter settings in Sacramento.

Key Responsibilities

  • Therapeutic Interventions — Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks under the supervision of the PT.
  • Participant Assessment Support — Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT.
  • Interdisciplinary Collaboration — Coordinate with nurses, social workers, occupational therapists, and other PACE team members to ensure integrated, personcentered care.
  • Home & Center-Based Care — Provide therapy services in participant homes, the PACE day center, and clinic environments as needed.
  • Equipment & Safety — Train participants and caregivers on safe use of assistive devices and mobility equipment; maintain a safe therapeutic environment.
  • Documentation — Complete accurate, timely electronic health record (EHR) documentation in compliance with PACE, state, and federal guidelines.

Qualifications

  • Accredited PTA Program graduate with an active California PTA license.
  • Current CPR/BLS certification.
  • Experience working with geriatric populations or in longterm care, home health, or PACE preferred.
  • Ability to work independently, travel locally, and adapt to changing participant needs.
  • Strong communication, documentation, and teamwork skills.

Schedule & Work Environment

  • Per diem / asneeded typically 8am to 5pm
  • Services delivered across Habitat Health PACE Centers (Sacramento, South LA, San Leandro, West Covina) participant homes.
  • Collaborative, mission driven environment focused on supporting older adults to remain safely in their homes.

Compensation:

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $40 - $46 per hour. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
