Permanent Senior Occupational Therapist
Clinical and Occupational Therapy Responsibilities
- Conduct independent OT assessments (functional, cognitive, psychosocial, behavioural, and environmental) to evaluate residents' strengths, risks, rehabilitation potential, and readiness for daily routines or work engagement.
- Develop individualised care plans to support participation in daily routines, structured activities, and purposeful in-house tasks. Provide OT interventions to improve functional performance, well-being, and quality of life.
- Assess suitability for internal or external Client's work opportunities. Support residents in building work readiness, confidence, task endurance, and independence through task matching, grading, and environmental modifications.
- Participate in MDT discussions, provide handover reports, and offer professional advice to nursing, care, and programme staff on safe and meaningful resident engagement. Support other Client's centres as needed.
Guidance to Therapy Aides (TAs) & Capability Building
- Guide and support Therapy Aides on safely engaging residents in therapeutic activities, structured routines, and work tasks.
- Provide TAs with practical approaches for activity grading, adjusting supervision levels, and managing residents with cognitive, behavioural, or functional challenges.
- Guide TAs on monitoring resident fatigue, mood, behaviour, and safety risks during tasks. Participate in case discussions and continuing education sharing sessions by the Welfare Home and/or Allied Health Services where required.
Documentation and Care Planning
- Document all assessments, intervention plans, progress notes, and outcomes in alignment with Client's requirements.
- Provide necessary documentation for internal/external referrals, care planning, and program development.
- Maintain detailed records of residents’ participation, functional changes, and recommendations for modifying or discontinuing activities.
Risk Assessment and Safeguarding
- Conduct comprehensive risk assessments for activity and work participation, identifying physical hazards, falls risks, cognitive limits, fatigue, and emotional readiness.
- Recommend clear risk mitigation measures, activity modifications, and strict supervision levels to ensure complete resident safety.
Quality Improvement and Outcome Monitoring
- Monitor resident progress in areas like task tolerance, functional ability, confidence, social engagement, and quality of life.
- Provide feedback to support clinical programme development and service improvement by the Welfare Home.
- Proactively participate in quality improvement projects, audits, and service initiatives driven by the Welfare Home and/or Allied Health Services where required.
Job Details
- Work Schedule: 5 days per week, 8 hours per visit
- Commencement Period: FY2026
- Senior therapist are welcome to apply
- Bachelor or Masters in Occupational Therapy
- Must be a Fully Registered AHPC ( Singapore )
