Pharmacist (Contract PRN)
The Pharmacist plays a vital role in advancing the community's health and wellness by ensuring safe, effective, and affordable access to medications. This position, working as part of a multidisciplinary care team within a Federally Qualified Health Center (FQHC), combines clinical expertise with a deep commitment to building trust, improving health literacy, and addressing social determinants of health.

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:
- Doctor of Pharmacy (PharmD) degree or B.S. from an accredited college of pharmacy.
- Active and in good standing Arizona pharmacist license.
- Two or more years of experience in pharmacy, preferably in a community health center or public health setting, with knowledge of the 340B program operations and compliance.
- Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if the applicant is from out of state) with no DUI/DWI or reckless driving convictions in the past five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the first ninety (90) calendar days of employment and maintain a valid card for the duration of employment with NATIVE HEALTH.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an up-to-date immunization (IZ) record upon employment.
- Must have a current Tuberculosis (TB) skin test report within the past 12 months upon employment and provide an annual update.
- Must obtain Basic Life Support (BLS) certification within 90 calendar days of employment and maintain certification during employment.
